We are currently seeking experienced and qualified candidates for Assessment Coordinator positions (formerly MMQ) at our facilities throughout Massachusetts. Key Responsibilities:

As an Assessment Coordinator, you will play a vital role in maintaining the quality of care in our facility. Your responsibilities will include:

UDA Generation: Generating UDAs (Uniform Data Set Assessments) prior to MDS ARD (Assessment Reference Date), with specific emphasis on GG (Section GG of the MDS) assessments. Monitoring and ensuring timely completion of all UDA’s. Documentation: Ensuring all UDAs are accurately filled out, locked, and completed on time. This includes assessments such as BIMS, PHQ-2-9, Pain Interview, Pain Assessment, Norton, Fall Assessment, Oral Assessment, and GG (Functional Abilities and Goals). Care Plans: Ensuring that care plans are present for all diagnoses for each resident, particularly when OBRA assessments are due. Medical Diagnosis Support: Verifying that diagnoses are present in the Medical Diagnosis tab in PCC (Patient Care Coordinator) to support the indication for medications the resident is on. Respiratory Care: Ensuring that there is a respiratory care plan in place, including the intervention "elevate head of bed to prevent shortness of breath as tolerated." Behavior Add-Ons: Assisting in documenting and maintaining documentation for behavior add-ons as needed. MDS Support: Performing MDS assessments as delegated to support MDS Coordinators as needed. Collaborating closely with the MDS Coordinator to ensure supporting documentation aligns with MDS coding. Team Collaboration: Providing coverage for other Assessment Coordinators or MDS Coordinators in case of vacations, illness, etc. Promoting teamwork and effective communication. Compliance Monitoring: Conducting daily monitoring of compliance with current and upcoming UDA requirements. Meetings: Attending various meetings, including Morning Clinical Meeting, Utilization Review, At-Risk/Behavior Meeting, and QAPI (Quality Assurance and Performance Improvement).

Qualifications:

Prior experience as an Assessment Coordinator or similar role in a long-term care facility. Strong knowledge of UDA and MDS assessments. Proficient in medical documentation and record-keeping. Excellent communication and interpersonal skills. Ability to work collaboratively in a team-oriented environment. Attention to detail and commitment to maintaining high-quality care standards.

Compensation:

LPN $40-$46/hr based on experience RN $44-$52/hr based on experience
