Care Coordination Assistant

Summary:

Care Coordination Assistant responsibilities:

Verifying insurance and benefits for all Medicare, Managed Care and Private Fee for Service Plans. Securing authorization for services provided by BayCare HomeCare from the payers. Selecting the correct payer codes to provide accurate intake information. Entering the appropriate financial information including benefits, deductibles, co-pays, coinsurance, out of pocket, another related costs into the EMR system. Responsible for timely request and communication of authorizations to payer and documentation in EMR. The Care Coordination Assistant will work under the direction of the Care Transition Coordinator or RN Case Manager to establish and maintain a positive working relationship with patients, payors and other customers.
