General Responsibilities:

Provides ongoing monitoring and care coordination for consumers on caseloadProvides training on disease management and healthy living skills according to the consumer’s target goals as identified by care planProvides care management services to consumers transitioning from more to less restrictive levels of careProvides support on behalf of the consumers in order to secure community resources and needed servicesEffectively collaborates and consults with staff members, including clinicians and pharmacists, and outside sources in the delivery and arrangement of care management servicesEvaluates and assists in consumer discharge planningDemonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situationApplies Mindset and restraint interventions per AltaPointe Policy and ProceduresProvides support and education for family members of consumersDemonstrates the ability to assess and reevaluate the consumer needs utilizing the CANS/SUNR, PHQ-9, DLA-20 (or approved assessments) as appropriateIdentifies and reassesses goals and plans for appropriate services utilizing the consumer’s care planProvides appropriate linkage to community resourcesDelivers or arranges clinically necessary transportationMeets AltaPointe productivity standardsEffectively demonstrates the application of child and adult specific competenciesMedical Monitoring as defined below:Basic knowledge of normal vital signs including temperature, pulse, blood pressure, and respirationsRecognizing and reporting vital signs outside of normal rangesRecognizing factors that influence temperature, pulse, respirations, and blood pressureKnowledge of operation of basic medical equipment used to measure vital signsRecognizing normal BMI for consumer’s height and weightRecognizing changes in nutritional status such recent weight loss or weight gainIdentifying factors that contribute to obesity and chronic health care conditions such as poor diet, inactivity and sedentary lifestyleEducate consumer on ways to adhere to prescribed dietIdentifying changes in normal nutritional statusDetecting changes in normal body functionsRecognizing and reporting changes in the consumers medical and mental statusBasic measurement of fluid balance (intake and output)Knowledge of operation of assistive devices such as wheel chairs, canes, and walkersAbility to recognize assistive devices in poor condition, in need of repair or replacementRecognizing signs and symptoms of acute medical conditionsRecognizing signs and symptoms of emergency medical conditions and when to call 911Identifying the seven rights of assisting with medication administrationSafely performs tasks within the individual’s scope of practiceProvide education in the maintenance of medications to include recognizing medications and understanding the purpose of medications, possible side effects and what to do when a dose is missed, extra dose taken, or side effects/adverse reaction occurProvide documentation of services rendered in the appropriate space indicated to include but not limited to consumer problem(s), interventions provided, and consumer response to interventions, referrals, collaboration with other care managers, follow up appointments, case management and community outreach services, etc.Documentation of assistance with emergency medications and proper reportingRecognizing signs and symptoms of abuse and neglect and proper reporting

 

Qualifications

Bachelor’s degree in a behavioral health or social services related discipline, LPN, Medical Assistant, or HSD/GED.

 

Completion of certification of case management training by Alabama State Department of Mental Health and experience within the Care Management field preferred

 

Knowledge in child and adult specialties, outlined below, to be attained within six (6) months of employment:

Knowledge of psychiatric disorders in child and adult populationsKnowledge of chronic medical conditionsBasic knowledge of growth and development of child consumers as related to all intellectual, emotional, spiritual, and physical aspects consumer developmentBasic knowledge of behavior management and parenting issuesKnowledge of cultural diversityKnowledge of behavior management, crisis intervention and interventions common to acute psychotic consumers, as well as, to non-violent crisis intervention practices.Knowledge of educational methods appropriate to adolescent/adult psychiatric disordersKnowledge of standards for the Alabama Department of Human Resources and Department of Mental HealthKnowledge of Alabama DHR and Medicaid billing requirementsKnowledge of social determinants of healthKnowledge of available referral sources for physical health, mental health and social services
