You then may want to be a part of our Transitional Housing Program in supporting those living with HIV impacted by co-occuring mental health and addiction.
In addition to other responsibilities, this position will have the opportunity to:
Complete an integrated assessment of individual client’s strengths, needs, abilities and preferences Develop relationships for transitional housing placementsDevelops an integrated and collaborative individualized care plan with a transition component. Provides the appropriate level of individual and group therapeutic intervention to support the stabilization and progression of clients Provides linkage and navigation for health care, supports services and housing stability based on individualized needsDevelops and prepares groups to meet client needs, including transition and after care.Provides supports to other disciplines in the residential program including but not limited to nursing, assessment, milieu and Behavioral HealthMeets with internal and external interdisciplinary treatment team members to coordinate client services and documents outcomes of meetings, including hospitals, health department and medical providers.Submits all documentation in an accurate and timely manner, including electronic records Achieves and maintains program established productivity levels.Engages clients in a strength-based supportive manner providing re-directions and guidance as necessary.Other duties as assigned. Case Management and Therapy experience preferred.Minimum of two years' experience which may include internships, preferredSubstance abuse treatment or residential setting experience preferred.HIV/AIDS knowledge preferredProficient computer skills.Bilingual preferred.Knowledge of Stages of Change, Trauma Informed Care and Harm Reduction Must have a Bachelor’s degree in Social Work, Health or Human Services from an accredited university or commensurate experienceValid Florida Driver's license and ongoing proof of insurance required and ability to provide own transportation.Ability to work independently with minimal directionCERTIFICATES AND LICENSESHIV 500 (provided upon hire)Valid Florida Drivers License and ongoing proof of insurance requiredHOPWA training modules (provided upon hire)
