Provides ongoing case management to assigned members. Bachelor’s degree in nursing, or the equivalent preferred. 2 years of experience in nursing preferred. License and current registration to practice as a Registered Professional Nurse in New York State required. UAS (Unified Assessment System) Certification required. NYS Driver's License required.
