Care Manager - Per Diem Job Details Level Entry Job Location 195 Montague Street, Brooklyn NY 11201 - Brooklyn, NY Position Type Per Diem Education Level 4 Year Degree Salary Range $30.00 Hourly Job Category Nonprofit - Social Services Description About the Care Manager Role:Graham Windham seeks dynamic health advocates to join our Children’s Health Home Care Management team. The Graham approach to Care Coordination is family focused and supports a strength based system of care. Our Care Managers will ensure continuity of care and comprehensive transitional care from service to service, across and within systems of education, foster care, juvenile justice and health and mental health care. Care Managers will also track clinical and functional outcomes. About the Per-Diem Care Manager Position: The Per-Diem Care Manager works alongside the Care Manager to assist in accomplishing specific objectives which must be in-place in order for Care Management services to be provided. This is a primarily field-based position and reports to the Program Director. RESPONSIBILITIES: + Conducts Home/Field visits to obtain necessary consents from the appropriate care-giver. + Works with the parent/care-giver to activate/re-activate health care insurance coverage, including accompanying the family members to HRA as necessary. + Conducting in-person visits to youth or family members as assigned by the Program Director. + Documents all efforts and submits copies to the Care Manager and Program Director.
