Care Manager RN, Case Management

Providence is calling a Care Manager RN, Case Management who will:

Comprehensively plan for case management services for a targeted patient population Carry out activities related to utilization management, discharge planning, care coordination and referral to other levels of care Work with physician, Social Worker and interdisciplinary team to facilitate clinical pathways and achievement of desired treatment outcomes Promote interdisciplinary collaboration and champion service excellence Work collaboratively to ensure patient needs are met and care delivery is coordinated across the continuum at the appropriate level of care

Please note the following important details regarding this role:

Work Status Available: Part Time Shifts Available: Day This is an onsite role that will report into the hospital daily, as scheduled

Required qualifications for this position include:

Bachelor's Degree - Nursing New Mexico Registered Nurse License Care Manager RN Auditor, Care Manager Clinical Audit and Denial, Care Manager RN Clinical Audit and Denial - Unit Educator, Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification - Within 2 years of hire date

Preferred qualifications for this position include:

3+ years direct patient care experience in an acute care hospital within a leadership role Case management or utilization management experience.
