Case Manager

Position Summary:

The Case Manager collaborates with WestCare staff and external bodies to provide assessment, counseling, re-integration, and/or rehabilitation services to program participants. Essential Job Functions include those listed below.

Essential Job Functions:

Provides case management services and assistance to program participants struggling with substance abuse, and those in recovery; Assesses the needs, conditions, and desires of assigned program participants; Educates and refers program participants to community agencies and businesses that appropriately address their needs for food, shelter, clothing, employment, transportation, etc. Assists participants with establishing or re-establishing community support systems; Acts as advocate on behalf of program participants to improve access to vital services; Participates in clinical staffing decisions regarding the participant’s current strengths and resources, and their need for additional resources; Completes documentation of services provided in a timely manner; Provides input into the patient’s treatment needs and disposition; Conducts assessments of program participants and provides therapy as needed; Reviews and documents all clinical services and activities; Participates in case conferences involving assigned program participants; Prepares Discharge Summaries, as needed; Assists other WestCare staff as needed to promote cooperation and portray a professional image to the community; Embraces and embodies the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation; Performs any other duties as assigned.
