In this role, you will collaborate with the person receiving services as well as their support team to develop and revise their goals. The Centralized Plan Coordinator ensures consistent implementation and documentation of all aspects of the person's program plan. This position primarily oversees the planning of services based on person centered planning concepts that are delivered to each person receiving services through Arc of Onondaga. \n

Job Requirements:\n\nBachelor's Degree in the Human Service field.\nOne year of experience working with individuals with developmental disabilities. \nMeet QIDP eligibility requirements.\nA valid NYS driver's license and a driving record acceptable to the Agency's insurance carrier.\n\n
