DUTIES AND RESPONSIBILITIES:
 

Communication and patient interaction:
1. Communicating with patients to gather information about medical history, concerns, requests for medication refills, or symptoms.
2. Providing clear instructions to patients regarding the next steps in their care plan under the guidance of the registered nurse or direct supervisor.
Coordination of Care:
3. Collaborating with the healthcare team to ensure timely and appropriate patient care.
4. Facilitating communication between patients and healthcare professionals, including documenting and relaying time sensitive messages.
5. Accurately documenting patient interactions and care recommendations under the guidance of the assigned registered nurse or direct supervisor.
6. Maintaining confidentiality of patient information and adhering to HIPAA regulations.
7. Ensuring completeness and accuracy of medical records to support the continuity of care.
Quality assurance and compliance:
8. Adhering to organizational policies, protocols, and regulatory requirements related to patient care.
9. Participating in ongoing training and professional development activities to stay current on best practices.
Clerical duties:
10. Scanning and indexing paper documents into the electronic medical record systems.
11. Ensuring proper organization and storage of electronic documents for easy retrieval.
12. Creating encounters, inputting relevant information into the encounter forms accurately and efficiently.
13. Recording and entering orders into the electronic medical system as directed by the registered nurse or direct supervisor. Verifying order accuracy and completeness before entry.
14. Scanning test results into the EMR system.
15. Uploading Holter and or pacer reports into the EMR system.
CMA / LPN duties:
16. Welcome patients by greeting them, in person or on the phone.
17. Verifies patient information by interviewing patient; reviewing and recording medical history and confirming purpose of visit.
18. Prepares patients for healthcare visits by directing them to the exam room, obtaining vital signs, performing medication reconciliation, and verifying patient medical information.
19. Assists in the care of assigned patients as directed by appropriate staff.
20. Ensure that patients receive appropriate competent, and quality clinical care and that customer service meets the patient’s expectations.
21. Following standard precautions using personal protective equipment as required.
22. Communicate all pertinent information, including findings, problems, concerns, or ideas to the assigned nurse, utilizing appropriate lines of authority, in a timely and effective manner.
23. Ensure all related reports, labs, and imaging are available within the patient’s charts prior to visit.
24. Performs his/her duties in accordance with the job requirements.
25. Seeks guidance and assistance from the assigned nurse when knowledge base and skill level is in question.
26. Performs tasks efficiently when putting charts together, stocking/cleaning rooms, stocking linens, placing Holter monitors, performing blood draws, and assisting with patient care.
27. Orients and mentors new staff members.
28. Participates in staff meetings, in-service programs, and education requirements as appropriate.
29. Is flexible and willing to assist with other duties within the clinic.
Performs other duties as assigned by department lead/supervisor as necessary.
