Position Summary

Assists in patient’s adjustment to their illness/condition and adherence to the treatment plan, as well as ensuring that they are aware of, and have access to, appropriate community resources. Includes patients/caregivers in the involvement or formulation and implementation of the patient's plan of care, and interdisciplinary collaboration will be employed to ensure the greatest continuity of care.

Primary Position Responsibilities

Provides social work services to patients and their families. Education and Counseling. Maintain resource materials. Documentation. Collaboration/Conference. Other duties as assigned.

Qualifications

Minimum Education: Required: Master’s Degree in Social Work Preferred: None
Minimum Years of Experience (Amount, Type and Variation): Required: Three years social work experience, preferably with older adult population; experience with community agencies helpful. Preferred: Three years social work experience, preferably in Home Health.
License, Registry or Certification: Required: New Jersey Licensure for Social Work, LSW or LCSW. Valid NJ driver’s license. Preferred: None
Knowledge, Skills and/or Abilities: Required: Strong clinical assessment skills and the knowledge and ability to respond to routine social work related inquiries. Knowledge of local community resources. Preferred: None
