Overview Ballard Rehabilitation Hospital is seeking Registered Nurses - for its San Bernardino, California worksite. The Registered Nurses will display an attitude of courtesy and respect for all residents, families, and staff; function as a team member with other staff in the performance of duties; assist residents with their activities of daily living, assist with mobility and transportation care; receive oral report from nurses on all assigned residents utilizing the unit assignment; report changes in resident's condition to RN/LPN in a timely manner; maintain accurate and complete records of food/fluid intake/output, baths, weights, and bladder elimination, ambulation, ROM and cleaning duties as assigned; coordinate the nursing care of residents on the unit ensuring all residents are assigned a nurse aide capable of providing care to the resident; provide safe and accurate medication related interventions to residents; administer and document medications and treatments according to each resident's medication schedule using current standards of medication pass technique; know the rationale for the effect of medications and treatments and correctly administer the same; provide treatments per the physician order including wound care, urinary catheterization, oxygen care, tracheostomy care, etc.; order and receive medications from the pharmacy; conduct and accurate accounting of all narcotics, signing for count and exchange of keys to medication carts and medication room; promptly respond to resident requests for pain intervention and documents such in the resident record; accurately transcribe and coordinate the implementation of physician orders; perform nursing assessments regarding the health status of residents; notify the physician of changes in status and promptly implement and document new orders; conduct ongoing and consistent assessments of residents to avoid unplanned hospital discharges by actively utilizing the Interact Program; develop a plan of care based on assessments, implementing nursing care; select and institute appropriate nursing interventions that might be required to stabilize a resident's condition and/or prevent complications; contribute to the resident's assessment (MDS/RAPS) and the development of a plan of care; consult with physician or licensed independent practitioner regarding general resident care needs; accurately report and document the resident's symptoms, responses, and status; design, promote, and organize resident education and counseling based on health needs; inform the resident and/or legal representative of incidents or changes. The successful candidate must also have Certificate from CGFNS, or a full-unrestricted state nursing license, or passage of the NCLEX exam.
