Ke Ola Pono Coordinator - Family Treatment Services (FTS) Honolulu, Hawaii

We are looking for a Ke Ola Pono Coordinator who will join our Housing and Case Management team on our Kaimuki Campus. Are you that individual?

Responsibilities:

Regular attendance is required for the position. This position will require some direct case management. Provides administrative duties for the Ke Ola Pono and outreach programs, which includes: Quality record audits to ensure proper documentation of client services that meets state and agency standards Reporting of program statistics to meet TSA and funder requirements (i.e. NSS, DHS monthly reporting and FTS internal attendance and census reports) Ensuring timely documentation for appropriate billing within the WITS system Coordination with the staff of Women’s Way residential, Outpatient Program Services and Ka ‘Ohu Hou O Manoa, in logistics related to client transitions through the FTS’ continuum of recovery Coordination with community organizations and other TSA programs (i.e. Pathway of Hope and Kauluwela Corp) providing special events and services for Ke Ola Pono Provides supervision and support in the following activities: Orientation of new residents to the program Weekly house meetings Random UAs and scheduling of UAs Documentation into WITS system Discharge of residents from program

 

Qualifications:

Bachelor’s degree in social work or related field with two years of experience in case management and coordination services, and/or outpatient substance abuse treatment services, or master’s degree with 1 year of post-graduate experience in case management and coordination services, and/or outpatient substance abuse treatment services preferred.  Candidates with lived experience and more than 5 years of work in the field are encouraged to apply. Personal vehicle and valid Hawaii State driver license required.

Salary range based on experience and education:

$49,000-$51,000 annually
