Licensed Practical Nurse Case Mgr

Pay Range:

$29.79 - $37.70

Provides professional nursing care to patients of all ages in a rural ambulatory care setting. Duties include the provision of urgent and emergent care in addition to well care and chronic disease follow-up. Schedule includes both regular clinic hours and on-call responsibilities. Initiates nursing actions and provides comprehensive nursing care to meet the physical, emotional, spiritual, and socio-cultural needs to the patient and family. Provides leadership to the nursing department.

Key Essential Functions and Accountabilities of the Job

Documents observations, nursing interventions, health teaching, therapeutic measures, and the coordinated activities between nursing and other professional disciplines.Observes, assesses, recognizes, identifies and interprets the patient’s condition.  Demonstrates ability to smoothly manage patient care duties while maintaining clinic flow.Develops an outpatient plan of care from assessment data for individual patients.Evaluates the extent to which nursing interventions have met the health care goals of the patient and revises the plan of care accordingly. Administers therapeutic measures as ordered by a physician or other qualified health care provider appropriates within scope of practice and department protocol.Coordinates Nursing Service activities for the clinic and with other departments.Plans and prepares staffing schedule in order to provide adequate support for physicians and nursing coverage for the clinical program needs.Conducts orientation for new nursing staff and ensures nursing competency documentation is completed.Provides guidance and leadership to other professional and paraprofessional nursing personnel.Develops departmental quality improvement studies and participates in clinic quality improvement activities.  Provides weekend, on-call, and secondary coverage of a moderate complexity lab.

Education, Certifications, and Licenses Required

Nursing degree – requiredAdvanced Cardiovascular Life Support (ACLS), Pediatric Advanced Life Support (PALS) Basic Life Support (BLS) certification – preferredActive nursing license in Alaska or other U.S. state required.Direct Hire must have applied for an Alaska nursing license at time of hire.Agency staff must have an active Alaska license.High school diploma or equivalent required.

Experience Required

Three or more years of general nursing experience in an acute care setting – requiredOne year of supervisory experience – requiredBackground in emergency / critical care nursing – preferred

Knowledge, Skills, and Abilities

Knowledge of:

Professional nursing care principles, practices and procedures; pharmaceuticals, their desired effect, side effects and the complications of their use; a wide variety of medical and psycho-social disorders, and the normal course of a disease.

Skills in:
