HeartShare is looking for a Licensed Practical Nurse (LPN)

As we grow our Integrated Health Services program to provide high quality medical health care to children in foster care under an Article 29I license, we are seeking a driven, committed, and experienced Licensed Practical Nurse ( LPN) who will perform health assessment and care coordination services to children in foster care. The LPN will provide assessment services to children in foster care in the support of pediatric care provided by Nurse Practitioner and Pediatrician and will also be responsible for the care coordination services.  They will report to the Nursing Supervisor (RN) and will receive clinical consultation support from the Medical Director.

 

Responsibilities:

Acting as liaison/point person to medical providers and pharmacies. Conducting assessments of children in foster care, including: HIV risk assessments, intake assessments, general first aid and triage activities. Develop a trusting caregiver relationship with youth and children in foster care. Routine screening for: substance abuse, developmental health, mental health and physical body checks for abuse. Routine management and training regarding chronic conditions, such as diabetes and asthma. Support and management of the emotional/psychiatric needs of children. Establishing treatment goals for children, utilizing historical and current available information in collaboration with multi-disciplinary team. Medication administration and management including reconciliation. Maintaining medical supplies and equipment. Practicing proper infection control. Follow up after medical appointments, urgent/emergency care or hospitalization. Educating caregivers and monitoring healthcare needs. Work with case planning team and health homes team to ensure high quality care coordination. Completion of Medication Reconciliation information for all children taking prescribed medication (psychotropic or otherwise) Ensure medication compliance as indicated by the ACS Medical Audit Mandates and American Academy of Pediatrics standards Support proper record keeping of all medical charts for children in general foster care and residential settings. Serve as back up to conduct as needed urgent physical screenings of children. Support in the completion of Adolescent assessments, sexual and reproductive health and family planning consultation. Gather family health information to complete pre-natal, family history and HIV assessment. Support in the gathering of consent for routine and urgent treatment, psychotropic medications, HIPAAs, and additional consents as needed. Documentation of all diligent efforts related to outreach to caregivers, youth and health care providers, or obtain documentation from case planners Communicate with health care providers as advocate and on behalf of clients. Monitor health care visits for children and adolescents in care based on the ACS mandates and American Academy of Pediatrics guidelines. Participate in case consultations with foster care, group homes/Youth Reception Center, and internally with Integrated Health Services department. Review of lab test when ordered by providers and update on results as needed. Document in a timely fashion all case contacts in Electronic Health Record (NextGen)
