Licensed Practical Nurse Navigator (LPN)- Neurology
Job Summary

This position will be a Licensed Practical Nurse performing duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the program. Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients. The Nurse Navigator, functioning in the spirit of teamwork, is accountable for the delivery of care utilizing skills learned in both the clinic and didactic arena. Assists other team members on the unit. The nurse navigator role provides clinical support to the provider and patients to assist in the diagnosis and treatment of conditions.

Job Duties Utilizes clinical knowledge to determine actions necessary to facilitate patient introduction to the program.Performs and documents evaluation of patients needs primarily by telephone or review of documentation.Effectively collaborates with other healthcare providers in coordinating and implementing the patients plan of care, to include evaluation, treatment, and surveillance.Effectively communicates with patient via mail, telephone, and in person regarding information pertinent to their care.Assures pertinent patient information is received and available for provider interaction with patient.Coordinates patient needs for the program.Serves as the point of contact for providers.Coordinates patient activities, such as support groups, educational series, as appropriate.Provides limited patient and family education based on individual needs either in person or by telephone.Coordinates patient activities through available support services, as appropriate.Maintains systems and processes sufficient to assure timely follow-up and surveillance of patients for both Programs.Serves as a liaison with Scheduling Services to ensure appropriate scheduling of patients.Develops and implements patient screening tools and efficient clinic procedures.Assists in the development and implementation of efficient patient navigational procedures.Investigates patient concerns including appointment access, communication, and billing.Maintains clinical supply inventory.Responsible for conducting medical record reviews to report findings to physician specialist.Provides nursing care and assists physician and procedures.Routinely inspects and observes condition and operation of equipment to ensure proper operation and identify the need for preventive and corrective maintenance per guideline.Performs ongoing maintenance of exam and treatment rooms.

Position Details

Hours: 40 hours per week; dayshift- hours are typically 8:00 a.m.- 4:30 p.m.

Weekends: None

On-Call: None

Travel: less than 50%- may need to travel to committee events, Geisinger sites that include DV and Scranton

Experience: minimum of 2 years prior LPN work experience is required
