The Medical Coordinator is responsible for coordinating medical and psychiatric services for our adults with developmental disabilities along with supporting them in order to help achieve their personal outcomes.

Responsibilities

Essential Job Functions
• Ensure that the rights, health, safety and personal outcomes of the people living in the residence are supported at all times.
• Coordinate and schedule all medical and psychiatric services for those we support including, but not limited to routine care, follow up care, and emergency care.

• Confirm location of appointment, obtain vehicle for appointment, obtain directions to appointment and allow ample time for timely arrival at appointment.

• Ensure that all needed information for successful completion of the appointment is in hand prior to leaving for appointment (i.e., consultation form, medical book, nurses notes, etc.)

• Obtains completed consultation form and needed scripts from the physician prior to leaving appointment.

• Review medical appointment calendar on a daily basis to see what appointments are scheduled for the day.
• Complete proper documentation of treatment and ensure this is communicated to all appropriate team members.
• Maintain medication inventory and ensure medications are ordered as needed.
• Ensure that all required documentation is completed /processed appropriately and in a timely manner.

 

Duties and Responsibilities

• Accompany and transport people supported on medical appointments and support person to advocate for their medical needs.
• Fulfill the essential job functions of a DSP as needed.

• Ensure the needs of the person supported are met before, during and after appointment. This includes but is not limited to hygiene, toileting, meals, snacks and medication administration.
• Assist medical staff in delivering in-service staff training.
• Always communicate in a respectful manner with all people including the people we support, supervisors, other valued team members, and members of the community.
• Complete all required trainings and stay in compliance.
• Maintain the confidentiality of any information relating to individuals and employees of the organization.
• Other duties as assigned.

Qualifications

Education, Work Experience, and Competencies
• 18 years of age or older.
• High School Diploma or GED required.
• Valid NYS Driver’s License and satisfy criteria to be a driver for FREE.
• Be able to accommodate flexible schedule based on needs of the program as needed.
• Demonstrated ability to work in a team environment with flexibility, keeping others informed and anticipating needs is essential.
• Must be efficient, well organized, and have excellent communication skills.
• Proficient writing skills with grasp of basic grammar and spelling.

 

Physical Demands
• Employee is required to sit, walk and stand for prolonged period of time (at least 8 hours).
• Employee is required to effectively communicate both verbally and in writing.
• Employee is required to operate standard office equipment such as computers, phones, photocopiers, fax and filing cabinets.
• Employee is required to reach with hands and arms.
• Employee is required to bend, stoop, kneel, crouch or stand on a stool as needed.
• Employee is required to be able to lift 50 pounds at a time.
