Position Overview
Overseen by Terry’s Place PROS Management, the Nurse Practitioner is responsible for evaluating the psychiatric needs of individuals, assisting with the development of an Individual’s Treatment Plan and to prescribe and oversee the medication regimen of individuals receiving clinical services with the program. The nurse practitioner will refer individuals for appropriate services as needed. The nurse practitioner will ensure that there is a team effort to provide overall high quality mental health services and work closely with all team members.

Responsibilities

Essential Job Functions
 Oversee a caseload of individuals
 Prescribe treatment appropriate to the admitted patients’ mental health problems, including medications as needed
 Assist staff in understanding the nature of the diagnosis and treatment provided
 Monitor the prescriptions provided to patients; review, as necessary, appropriate laboratory values; make adjustments to prescription medications as needed
 Attend clinical team meetings as scheduled with the PROS staff
 Maintain patient’s right to confidentiality at all times
 Ensure that all required documentation is completed /processed appropriately and in a timely manner and as per Quality Assurance protocols
Duties and Responsibilities
 Cooperate with all members of PROS staff to ensure that the patients’ overall health needs, both physical and mental are being met as needed
 Conduct meetings with other staff members as needed to ensure that there is a well-coordinated care plan among all disciplines providing services to a patient
 Demonstrate positive interpersonal skill in all interaction with patients, caretakers, peers and management
 Address emergency situations as they occur in the program according to established policies and procedures, and ensure communication of such situations in a timely fashion
 Provide coverage in emergency situations until relieved of the responsibility
 Complete psychiatric evaluation reports for patients as needed
 Interview referred patients and other individuals familiar with the patient in order to diagnose mental health problems
 Participate in internal and external committees, groups and stakeholder meetings as needed
 Document assessments and responses to treatment appropriately on the medical record; write reports as necessary
 Ensure that input to electronic patient records is kept current including an up-to date care plan
 Handle records in a professional manner to protect patient and employee confidentiality and as per HIPAA guidelines
 Other responsibilities as assigned

Qualifications

Minimum Job Qualifications
 State Licensed and Board Certified
 1 year experience working with mental health population (inclusive of prescribing and monitoring of psychiatric medications) with some generalist/primary care experience
 License actively registered and current with the Office of Professions
 Active NPI with Medicaid and Medicare privileges
 Active DEA Registration
Physical Demands
 Employee is required to sit, walk and stand for prolonged period of time (at least 8 hours)
 Employee is required to effectively communicate both verbally and in writing
 Employee is required to operate standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines
 Employee is required to reach with hands and arms
 Employee is required to be able to lift 25 pounds at a time
