**This is a full time position (30-35 hours/week) float position supporting our clinics in the San Antonio area.**

POSITION SUMMARY:   

CVS Health is seeking a Nurse Practitioner (NP)/ Physician Assistant (PA) to join our Primary Care team. In this capacity, you will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients.  This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This role will report to the practice manager.

PRIMARY DUTIES AND RESPONSIBILITIES:

Responsibilities include but are not limited to

Patient-Centered Quality and Safety

Uses knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional to provide care to patientsPerforms history and physical assessments of patients that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical symptoms.Provides patient counseling; inclusive of pregnancy prevention, STI Prevention/safer sex practices, contraceptive care counseling and medication managementCounsels and educates patients and families on health maintenance and respond to patient care inquiriesEvaluates and treats patients' medical needs with safe and high-quality treatmentDocuments all patient care within an electronic health record in accordance with current policies and proceduresWorks as a member of the clinical care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is always provided for all patientsProvides patient-centered care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of disease Refers to appropriate specialists and patient care components as appropriateMaintain patient confidentiality in accordance with PHI and HIPAA standards

EDUCATION: 

Completion of master’s degree level Family Nurse Practitioner or Physician Assistant program with current National Board Certification and State of Employment license to practice in the Advanced Practice role required.

ADDITIONAL REQUIRED SKILLS AND ABILITIES: 

Active Advance Practice Registered Nurse, Nurse Practitioner license or Physician Assistant license in good standingCurrent National Board Certification, Family PracticeA minimum of 1 year of recent Primary Care experience as a Nurse Practitioner/Physician Assistant in either a family medicine or primary care practice preferred. If not, successful completion of CVS Minute Clinic primary care training program.Working knowledge and understanding of quality measurement in the management of chronic disease conditionsSelf- motivated, prioritizes and solves problems, takes initiative, and advocates for their patients and their practiceEffective verbal, written, and electronic communication skillsOutstanding organizational skills and ability to multi-taskInitiative, problem solving ability, adaptability, and flexibilityAbility to work remotely in a clinical care team cultureAbility to work without direct supervision and practice autonomouslyIs proficient with information management and technologyWillingness to obtain multi-state licensuresCapacity to collaborate with professional colleagues frequently to ensure quality patient care

WORKING ENVIRONMENT:

While performing the duties of the job, the Primary Care NP/PA is regularly required to interact with patients remotely, sit, write, operate the computer and phone, speak intelligibly, and hear patient responses Specific vision abilities include the ability to view and read a computer screen and other electronic devices  

Pay Range

The typical pay range for this role is:

$37.66 - $81.11

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
