Pharmacy Resident PGY-1

The Pharmacy Resident PGY1 is responsible for working with multidisciplinary health care teams to achieve optimal outcomes. Compounds and dispenses medications and pharmaceutical supplies upon receiving physician orders. Reviews and evaluates drug regimens, which include the medication profile to identify issues that require pharmacy intervention or monitoring. Provides pharmacy services and drug information to customers as necessary.

GENERAL DUTIES

Prepares, dispenses, and manages medications to support safe and effective drug therapy for patients. Instructs patients in the proper use of prescribed drugs and makes patient care rounds with physicians to evaluate patient progress.Conducts and evaluates medication histories, assesses compliance and suggests modifications so as to achieve desired outcomes.Collaborates with the healthcare team to provide safe and effective patient care to a diverse range of patients, including those with multiple co-morbidities, high-risk medication regimens and multiple medications following a consistent patient care process.Participates in collection of data for drug utilization evaluations (DUEs) and medication education to in-patients/out-patients.Residents are expected to complete a research project by demonstrating the ability to evaluate and investigate practice, review data, assimilate scientific evidence to improve patient care and/or the medication use system.Provides general and patient specific drug information to patients, physicians, nurses and other health care professionals. Interacts routinely with physicians and nurses to provide optimal pharmaceutical care to patients.


EXPERIENCE QUALIFICATIONS

3 years of experience in Clinical/Direct Patient Care. (TOURO/EJGH)


EDUCATION QUALIFICATIONS

Required: Graduate of an ACPE accredited Doctor of Pharmacy degree program.Required: Bachelor of Science in Pharmacy.Required: Master’s Degree or significant clinical practice experience (e.g., minimum of 3 years of clinical/direct patient care)


LICENSES AND CERTIFICATIONS

Certification Name: Pharmacist InternRequired prior to start of program.Issuer: Louisiana Board of Pharmacy

Certification Name: PharmacistRequired before October 1 of the year starting the residency.Issuer: Louisiana Board of Pharmacy
