Job Title: Psychiatric Registered Nurse, Young Adult Assertive Community Treatment (YA ACT) Team

Reports to: Team Leader, YA ACT

Location: New York City

Salary: The salary for this role is $95,000 per year.


Overview:

The Young Adult Assertive Community Treatment (YA ACT) team is a multi-disciplinary team that provides evidence-based, mobile, community-based mental health treatment to promote the recovery and community integration of individuals age 18 to 25 with serious mental illnesses, high rates of co-occurring substance use disorders and high utilization of emergency/crisis services. The team also provides developmentally appropriate support to build and implement productive vocational or education plans, and will focus on enhancing necessary skills for independent living and self-management.

The Registered Nurse provides health assessment, counseling, psychoeducation, and medication management to promote recovery, wellness, and optimal health. The nurse provides at least 80% of services in the community within the framework of core ACT operating principles, professional nursing ethics, and regulatory guidelines.


Responsibilities:

· Provides medication management and health services to consumers in the community, including but not limited to: medications administration (both oral and intramuscular), monitoring medications side effects and adherence, phlebotomy, coordination of consumers’ health care services, providing psychoeducation to consumers about health and medications, assessing consumers’ mental statuses, rotating on team’s after-hour emergency line;

· Completes and signs progress notes, nursing assessments, incident reports, and other required documentation in accordance to the New York State Office of Mental Health (OMH) regulations, the New York City Department of Health and Mental Hygiene (DOHMH) guidelines, and Medicaid billing standards and regulations;

· Coordinate with YA ACT prescriber on consumers’ medication management needs, health care services, maintaining consumers’ medication administration schedules;

· Coordinates care with family and other community service providers or collaterals including, but not limited to: hospitals, housing providers, shelter staff, criminal justice personnel, Assisted Outpatient Treatment (AOT), and primary health care providers;

· Participates in team meetings and clinical meetings to collaborate and discuss clinical issues and clients’ needs with other team members;

· Use motivational interviewing, culturally and developmentally appropriate, recovery oriented and trauma-informed approaches when delivering services;

· Any other duties as required by Team Leader and senior clinical leadership.


Qualifications:

· Registered Nurse (RN) with current New York State license

· At least three years of experience working with young adults (ages 18 to 25) in one or more of the following areas: psychiatric nursing, substance use treatment setting, social service setting(s) caring for individuals experiencing homelessness, justice involvement or limited social support;

· Extensive knowledge of community treatment, support services, and resources

· Highly organized and self-motivated

· Proficient in computers

· Fluency in Spanish preferred
