Position Summary:
We have an exciting opportunity to join our team as a Registered Nurse- Coordinator-Complex & Chronic- Full Time, Days

In this role, the successful candidate is responsible for coordination and continuity of Healthcare as patients with complex and chronic conditions are transitioned into the community. Collaborate with various department and multidisciplinary teams to develop and implement strategies
to assist patients throughout care episodes, coordinating and facilitating care for patients. This position provides services
to support expected program outcomes of reduced length of hospitalization, readmissions, emergency room visits,
improved medication compliance, reduced gaps in care, improved patient care coordination, and increased patient
satisfaction with the health care experience.

Job Responsibilities:

Assess readmitted patients through patient caregiver interview and medical record review for contributory factors,
including gaps in transition process, which will be communicated to the provider and the multidisciplinary team.
This information will be tracked for trends. Performs other duties as assigned Review program for opportunity to improve outcomes. Collaborate with other departments, participate in planning
and implementation of evidence-based practice and care models related to patient-centered coordination across
the continuum. Maintain data to measure program success. Utilize data indicating value of the program to engage stakeholders
including physicians. Maintain contact with patients identified as high risk for readmission in all post-acute care settings by phone or site
visit as indicated. Engage patients in taking a proactive role for managing their health, medications, treatments,
rehabilitation and follow up appointments. Encourage use of in network services. Collaborate and communicate with the multidisciplinary team and internal and external stakeholders including
post-acute care facilities, home care providers, and physician offices to develop and implement strategies to
ensure continuity at transition points. Provide education as indicated to ensure best practice. Engage staff in the education process of patients caregivers. Coordinate an ongoing evaluation of the teaching
tools and methods utilized including patient education handouts and teach-back techniques. Serve as a resource
and mentor to staff in effective patient caregiver engagement and education. Collaborate with the multidisciplinary care team to ensure compliance with provider utilization of standard order
sets, discharge instructions and adherence to associated policies and procedures including those for workflow and
consistent documentation.

 

Minimum Qualifications:
To qualify you must have a Bachelor's Degree Nursing.

Current NYS Registered Professional Nurse License.

3 - 5 years Acute Care clinical patient care experience.

Excellent communication skills (both oral & written), interpersonal and organization skills.

Proficiency with computers to include Microsoft Word and Excel.

Ability to foster strong collaboration with co-workers, peers, physicians, nursing, physicians offices and ancillary departmental support staff.

Required Licenses: Registered Nurse License-NYS

Qualified candidates must be able to effectively communicate with all levels of the organization.

Long Island Community Hospital provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $77,000.00 - $130,250.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
