POSITION SUMMARY

The Community Wellness Nurse is a clinically competent, professional registered nurse who prescribes, delegates, and coordinates nursing care to their specific patient population through the nursing process of assessment, planning, intervention, implementation and evaluation.  Provides leadership and coordination for the clinical operations and patient care coordination.  Responsible for the development and implementation of processes that facilitate patient care, care coordination, quality of care and achieve desired programmatic goals in collaboration with physicians and hospital leadership.  Works with leadership team to establish, implement and monitor programs.  Also directly works with patients to assist in adhering to their plan of care and promote delivery of excellent service and high quality clinical outcomes.  Responsible for growth of programs and achievement of financial targets set for programs.  This individual will lead the hospitals’ efforts in development of discharge planning and after discharge follow up for patients. This individual will work collaboratively within the Health System with clinicians, medical staff and support services.  The primary role will support care delivery and the development and utilization of protocols aimed at clinical quality excellence through the use of evidence-based medicine and adoption of best practices.

Key function areas are: (1) clinical quality, (2) best practices, (3) patient education, and (4) community engagement.

JOB REQUIREMENTS
 

1.   Graduate of an accredited school of nursing with a bachelor’s degree in nursing, health education, or related field.

2.   Preference of experience in a faith based health system

3.   Minimum of three years’ clinical nursing experience and one year of community-based experience  preferred.

4.   Must be self-directed and able to work with little supervision.

5.   Currently licensed to work as an RN in the state of South Carolina

6.   BLS/CPR certification, Current SC driver's license
