We are hiring a Resident Services Coordinator II to bring their passion for resident engagement into our community.

Compensation: between $55,000 - $60,000 per year commensurate with experience.

How you will make an impact:

Responding to residents in need of psychosocial or supportive services, through direct referral or by the review of incident reportsConducting assessments of potential residentsRecommending appropriate levels of residencyAssessing and resolving issues regarding residents’ care and safetyDeveloping referral networks within the community for available servicesEducating residents in the importance of Advanced Directives and completion of documents

What you will need:

Bachelor’s degree in Social Work or related field required.Minimum of 3 years of social work experience in a health care setting preferred.Excellent verbal/written communication skills.Knowledge of geriatric population and dementia preferred.
