Position Overview

Under the supervision of the Assistant Vice President of Self-Directed Services, the Self Direction (SD) Coordinator, will provide support and guidance to individuals who self-direct their services through a self-directed budget under the auspice of OPWDD using a person-centered approach

Responsibilities

Essential Job Functions

Conduct/participate in Launch Meetings, Life Plan and Circle of Support meetings and assisting individuals and their families with billing, training, staffing and other service related concerns.Access training resources for individuals, families and staff.Reviews, approves and submit to OPWDD participants Self-Direction Budgets;Review and submit participants invoices for reimbursement and payments;Work with families, brokers, and vendors to ensure self-hired staff clear all required background checks.Assist individuals and service providers to complete required service documentation;Review service documentation to ensure it is complete, accurate and in compliance with all OPWDD, Medicaid and FREEs quality standards.Seek input from individuals and families regarding job performance of and satisfaction with self-hired staff.Work with the Human Resources team to ensure self-hired and agency staff receive required training and any additional training requested by the individual and his/her family or circle of support.Ensure monthly expenditures are accurate and submitted for reimbursementAssist Brokers to help individuals and families understand and fulfill their employee and service related responsibilities.Maintain records of contacts with people including but not limited to: individuals, family members, To be the primary point of contact and source of customer service for individuals and their families;Review and submit documentation completed by support brokers on behalf of the individuals they are supporting, including but not limit to, Startup Budgets, Staff Action Plans, Life Plans, Level of Care Documents, Full Budgets, etc.Work with individuals receiving services, brokers, state agencies, and families to ensure the individual is receiving all services requested;Review and monitor participants monthly expenditure reports; Qualifications

Education, Work Experience, and Competencies
