What’s a Service Coordinator?

Service Coordinators are integrated into senior housing to help residents connect with community resources, agencies, and other service providers. They also serve as resident advocates who empower seniors to live more independent and fulfilling lives. They refer residents to providers and monitor how well those services are provided.

In this role, you will:

Provide case management and referral for up to 100 residents. Conduct needs assessments. Develop service management plans. Organize on-site educational programming.

What we are looking for:

We need Service Coordinators who:

Have a bachelor's degree (Social Work, Gerontology or a related field a plus!) OR four years related experience. Have a valid Pennsylvania driver’s license and access to a registered and insured vehicle for frequent and regular travel between communities. Are able to effectively interact with residents and team members.
