Responsibilities Stella Maris is looking for a Social Worker to join our Inpatient Hospice team. The main function of this position is provide social work services to our Inpatient Hospice patients and their families. JOB RESPONSIBILITIES: • Services include admission, psychosocial assessment, plan of care, linkage with community resources, counseling, crisis intervention, and discharge planning. • Responsible for required documentation in compliance with State and federal regulations. • Assesses bereavement issues of families and makes timely and appropriate referrals to Center for Grief and Loss. • Facilitates effective communication between patient/family and home care team members. • Provides appropriate information and referral services to patients and families/caregivers. • Assumes responsibility for discharge planning as needed Requirements QUALIFICATIONS: • Master’s Degree in Social Work • Social Worker License in Maryland • Experience in hospice or medical setting preferred
