Supervising Habilitation Specialist - STA010 - Mon-Fri 8:00AM-4:00PM Job Details Job Location Station Road Day Hab - Flushing, NY Education Level 4 Year Degree Salary Range $22.19 - $22.19 Hourly Job Description Position Summary:The Supervising Habilitation Specialist is responsible for supervising the daily operation of assigned units. This includes training and supervising assigned Habilitation Specialists and Assistant Habilitation Specialists, designing, implementing, and reviewing individual program plans and daily activity schedules, and serving as QDDP (Qualified Mental Retardation Professional) for assigned caseload. Essential Accountabilities: + Responsible for ensuring that all activities of assigned units enhance and support each persons social responsibility, independence, dignity, and continued growth. + Participate in interviews and recommend for hire applicants for Habilitation Specialist and Assistant Habilitation Specialist positions. + Provide ongoing support and supervision to Habilitation Specialists and Assistant Habilitation Specialists to ensure each participant receives ongoing quality programming. Supervision will include one to one meetings: direct monitoring in the units and in the community, and review of all assigned paperwork. + Provide ongoing feedback and training to Habilitation Specialists and Assistant Habilitation Specialists. Institute re-training as necessary. + Responsible for conducting performance appraisals annually and as needed for Habilitation Specialists and Assistant Habilitation Specialists. + Serve as QDDP for assigned caseload to ensure each individual program plan is developed and implemented as per recommendations of interdisciplinary team. + Responsible for the planning, writing, and implementation of goals designed to maximize each participants level of independence, as determined by the IDT. + Establish and ensure appropriate activity schedules are implemented for assigned caseload. + Monitor participants progress by observing, documenting, and analyzing performance. + Collect annual, semi-annual, and monthly data/progress for all programs, review, and make recommendations for continuance, revision, or discontinuance of goals. + Complete all documentation and required paperwork including Annual; CFA. Semi-Annual, monthly review of goals and, data, including clinical data, within specified time frames. + Participate in all Interdisciplinary Team Conferences for all assigned participants either at the day program or residence. Act as the liaison between the two programs, ensuring that all information is forwarded to the appropriate person(s). + Communicate with residences and/or families regarding progress, incidents, etc. Act as a liaison for all community contacts and program staff. + Serve as a positive role model for participants and staff members in a variety of program and community settings. + Provide behavioral interventions when necessary, following the least restrictiveOPWDDapproved techniques required by the situation. + Ensure that assigned units are clean, safe, stimulating, and appropriately decorated. + Review time sheets of Habilitation Specialists and Assistant Habilitation Specialists and verify hours of work, and approve or deny requests for time off, in conjunction with Program Coordinator, and needs of the program. - + Responsible for the overall functioning of the program as assigned in the absence of the Program Coordinator. + Participate in and attend meetings/training at various sites. Participate in Agency and Day Habilitation Program Committees, as assigned. + Participate in all routine program functions such as fire drills, mealtimes, arrivals and departures. Participate in various appointments (medical, etc.) for participants, as required. + Must be able to physically interact with participants, negotiate the environment, including performing all SCIP interventions, emergency CPR, AMAP, and or first aid techniques and fire drills. + Maintain confidentiality at all times. + Caseloads and locations are subject to change. + Other duties as requested Qualifications Bachelors degree required in Education, Human Services; or related field. Valid NYS drivers license and two years experience working with people with developmental disabilities preferred.
