We are looking for a Support Counselor in ICM who will provide behaviorally focused mental health services to children and families. As the ICM Support Counselor, you will provide community resourcing, advocacy and safety planning and provides to youth and their families referred to the ICM program. As a Support Counselor, you also will develop tailored behavioral interventions to support clients and families reach treatment goals and prevent further contact with the Juvenile Justice System and/or to successfully reintegrate back into the community.

Intensive Case Management (ICM) services are designed to support youth's current living environments and mental health while also linking youth and families to mental health, educational, and community resources in order to increase functioning. These services, which are child and family focused and strength based, are provided in the office and community, including, but not limited to, schools, hospitals, and family homes.

Responsibilities Provide culturally responsive behavioral support services to youth and their families referred by Sonoma County Juvenile Probation. Collaborate with ICM Facilitator, Family & Peer partners to develop individualized treatment plans, interventions, and action plans to support a young person’s journey to diversion from further system involvement. The ICM Support Counselor will conduct observations and assessment of youth needs, concerns, and strengths in order to contribute to service planning and development of individualized care. Work collaboratively with caregivers and other family members to identify and practice helpful parenting skills. Complete documentation of mental health notes and other clinical documents in a timely manner and in accordance with program guidelines. Co-facilitate and/or participate in monthly child and family team meetings for each assigned youth.  Provide youth and families with linkage to community resources or community providers to reinforce sustainable solutions. Participate in family finding or natural support team building for families and youth.  Qualifications Experience/education requirements:Bachelor’s degree ORAssociate’s degree or equivalent + 1 year of applicable work experience ORHigh school diploma/GED + 2 years of applicable work experienceApplicable experience includes any paid or unpaid experience working with youth or families, or working in the social services fieldBilingual in Spanish/English preferredExperience providing strong behavioral interventions with youth, preferably in a community-based setting Valid California driver's license and insurable by Seneca's automobile policyClearance of TB test, fingerprints, and any other mandatory State/Federal licensing or certification requirementsProof of COVID-19 vaccination and booster required Schedule Full-time - 9:00 am - 5:30 pmTuesday-Saturday Flexible schedule needed with ability to work evenings and weekends Benefits Starting at $22.66- $25.16 per hour.
