Job Description:

SUMMARY OF POSITION

Provides day to day supervision and living skills instruction for the residents and carries out duties as assigned by the Program Manager, Program Manager Assistant or TLP Advisor Team Lead.   This includes providing training and support for residents to help them attain maximum independence and maintenance of appropriate documentation. 

ESSENTIAL DUTIES & RESPONSIBILITIES

Assist service recipients with making linkage with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services.  And maintain appropriate contact with referral sources including providing information on service recipient progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate. Provides transportation for service recipients as needed.Provides instruction regarding personal care as needed.Assist client with life skills as needed, including but not limited to: assisting client with cooking, cleaning (to maintain DCFS/DJJ, Centerstone cleaning standards. If the client refuse to clean, staff must ensure that client environment is up to standards by cleaning and getting back in order, budgeting, interpersonal skills, resume building, vocational skills and other life skill instruction.

QUALIFICATIONS

Education Level              

Associates Degree plus one year of mental health experience. If applicant has no degree, five years of direct work experience with mental health population is mandatory. TB testing also required.

Certification/Licensure 

CPR/1st AidCPI
