JOB SUMMARY

Leadership: Utilizes problem solving skills to identify and work through problems. Maintains standards of care by collaborating with physicians, managers, and other clinical team members to ensure cost effective, quality care based on the patient’s psychosocial status, physical limitations, abilities, and support system. Provides education and guidance for other healthcare providers utilizing expertise in the field.

Planning: Collaborates with the interdisciplinary team, patient, and caregivers to identify individualized needs and preferences and assist with the development of the plan of care. Collaborates with physicians, managers, clinical coordinators, DME companies, home health agencies and medical supply companies in the development of the plan of care. Identifies patient and caregiver learning needs and incorporates into plan of care.

Implementation: Assist provider with bedside procedures such as surgical debridement, preparation and application of skin substitutes and biopsies. Utilizes critical thinking skills in the application of advanced wound care dressings, multilayered compression bandages, orthotics, and lymphedema devices. Provides ongoing education and teaching regarding dressing applications and techniques for home use if applicable. Provides ongoing teaching and evaluation of the use of DME such as application and maintenance of compression devices. Provides ongoing education specific to the specialty to promote improvement in health and wellness. Prioritizes job duties according to patient needs and acuity levels. Analyzes, modifies, and implements strategies to progress the patient through their plan of care. Demonstrates expertise in patient transfer techniques and use of patient lift. Monitors care to ensure compliance with policy and procedure. Generates referrals for interdisciplinary care such as medical supply orders, DME, podiatry, infectious disease, vascular, imaging, and plastic surgery. Assures all orders are completed in a timely fashion. Continually assessing plan of care and collaborating with provider regarding progress toward goals and outcome. Reassessment of wound status with each clinic visit for plan of care modifications.

Delegation: Educates and orients new team members and serves as a resource person and role model with expertise in the specialty. Assists clinical supervisor assuring clinical competency of clinical staff. Demonstrates a commitment to quality by holding others accountable for quality care.

Teamwork Utilizes data from patient satisfaction surveys to identify opportunities to improve and participates in planning and improvement processes. Responsible for maintaining all clinical JCAHO standards.

This position will facilitate projects and tasks to promote the health of our patients in the community.  

 

Patient Care: Acts with patient advocacy when implementing the interdisciplinary plan of care. 

Collaborates with healthcare team to educate patient and their support system.   Analyzes, modifies, and implements strategies to progress the patient through their plan of care.   Ensures completion of nursing interventions, medical orders, and other related therapies.   Prioritizes job duties according to patients' needs and changing acuity levels. 

Communication: Consistently utilizes appropriate communication tools when communicating within the interdisciplinary team and when handing off patient care.  

Communicates individualized plan of care to the patient and his/her chosen support system.   Uses communication tools such as Situation, Background, Assessment, Recommendation, and Question (SBARQ) to communicate more effectively.   Documents appropriate information in a timely manner. 

Teamwork: Develops and maintains positive relationships with team members/internal customers to provide efficient treatment of patients. 

Develops team members through mentoring, precepting, orientation, and providing feedback.   Resolves conflicts independently, as appropriate.   Accepts responsibility for actions and responds positively upon feedback from others. 

Medical Knowledge: Demonstrates medical skills to provide required patient care. 

Operates various equipment including portable cardiac monitors, bedside commodes, etc.   Provides routine nursing care such as starting IVs, performing dressing changes, etc. for multiple patients in a timely manner.   Assists in critical situations including administering CPR.  Administers and evaluates medication therapies, performs patient transfers, and transports patients via wheelchair, hospital bed, or stretcher to other departments. 

The incumbent may be asked to perform additional duties as assigned.

 

QUALIFICATIONS

Education: 2 Year/Associate's Degree required. 4 Year/Bachelor’s Degree preferred.

Experience: 2 years of experience required. Refer to the Life Support Training Policy for additional details.

Licensure: Current RN licensure in applicable state required. Wound Certification preferred

Additional Skills/Requirements Required: Excellent oral and written communication, problem-solving and critical thinking skills. Strong interpersonal skills. Must be able to provide care with minimal supervision.

Additional Skills/Requirements Preferred: Expertise with advanced wound care products and techniques.

The job requires constant standing, frequently walking, and bending/stooping. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment.
